Clinical Nurse Specialist outpatient clinics:
Our Clinical Nurse Specialist (CNS) Clinic is an outpatient service for people living with a life-limiting illness who would benefit from regular support, but may not wish to attend Day Hospice.
It allows you to stay connected with Primrose Hospice and receive expert advice, support and care through planned appointments.

How our Clinical Nurse Specialist can support you:
Our Clinical Nurse Specialist is a highly trained nurse with expertise in palliative care.
They provide personalised support to help you manage your illness and feel more in control.
They can help with:
– Managing symptoms such as pain, fatigue, or breathlessness
– Providing emotional and psychological support
– Answering questions about your illness or treatment
– Helping you plan for the future and make informed decisions
– Coordinating care with your GP and other healthcare professionals
You’ll have time to talk openly in a private, supportive setting. Carers or family are welcome to join clinic appointments.
Who is the Clinic for?
The CNS Clinic is suitable if you:
– Are living with a life-limiting illness
– Would like regular support from a specialist nurse
– Prefer one-to-one appointments rather than group settings
– Want advice on managing symptoms or planning your care
It can be used alongside other hospice services if needed.
What to expect at your appointment?
At your clinic appointment, you will meet with our Clinical Nurse Specialist in a private consultation room.
During your session, you can:
– Talk through any symptoms or concerns
– Ask questions about your illness
– Discuss your care options and preferences
– Receive advice and practical support
Appointments are tailored to your needs, and you can attend as often as agreed with our clinical team.
We understand that everyone’s needs are different.
Some people prefer one-to-one support rather than attending group services like Day Hospice. Our clinic offers a flexible, supportive way to access expert care in a calm and private environment.
How to access our Clinic:
To attend our Clinic, you will need to make a referral. We are able to support patients who are registered with a Bromsgrove or Redditch GP.
You can refer yourself by completing our online referral form here or calling us directly: 01527 871051
You can also be referred by:
– Your GP
– Hospital consultant
– District nurse or healthcare professional
What happens next?
After we receive your referral:
– Our team will contact you to discuss your needs
– We will explain the support available
– You may be invited to meet a member of the team
– Together, we will agree whether our Clinic is right for you
You will always have time to ask questions and decide at your own pace.

Do I have to attend Day Hospice to get support?
No, the clinic is an alternative for people who prefer one-to-one appointments. You can attend Day Hospice and Clinics whilst you are accessing support at Primrose Hospice.
How often will I have appointments?
This depends on your needs and will be agreed with your nurse.
Can I access other hospice services as well?
Yes, we can connect you with other support if appropriate.
Is the service free?
Yes, all Primrose Hospice services are free of charge. If you would like to make a donation, please visit this page.

Medical clinics
The Palliative Care Consultant is employed by Hereford and Worcestershire Health and Care NHS Trust and is based at the Princess of Wales Community Hospital.
Outpatient appointments are carried out in a variety of settings such as the Primrose Hospice Day Hospice and the Princess of Wales Community Hospital, following a direct referral from the Hospice Clinical Team, GP’s and hospital Consultants.
If you are known to one of the hospice team, the consultant will see you with a member of the Clinical Team at the Day Hospice.
