Make a referral to our Family Support Services

Our Primrose Hospice Family Support Centre is here for you and your family to support you through what may be a difficult time. We offer a range of one to one sessions and group support for all ages.

Our friendly Family Support team will provide you with support and advice for all your family to deal with difficult situations or decisions.

To be referred to the Family Support Service, please contact your GP or contact the service directly, between 9am and 5pm, and ask to speak to the Family Support Team Leader.

Alternatively, you can ask any education, social care or healthcare professional to make a referral on your behalf.

Bereavement support is available to anyone registered with a Redditch and Bromsgrove GP who have experienced a bereavement in the last three years or who has previously been supported by Primrose Hospice.

What happens next?

A member of the Family Support team will be in contact with you once a referral has been made. We aim to arrange an assessment for you within two weeks and from this we will agree which services we consider would be most helpful to you.

There is sometimes a wait for some of our services e.g. counselling or some of our specific group support, but your name will be placed on a waiting list and we will contact you as soon as we can.

How can I make a referral to your Children & Young People’s services?

To accept a referral we would require the following information:

  • Name of Child
  • DOB
  • Address
  • GP
  • Parent/Carer’s name
  • Brief information regarding the child’s situation
  • Referrer’s details

Please note:

  • Parents/carers must be aware of the referral
  • We aim to make initial contact with the family within 10 working days
  • Children can be assessed with their family or on their own.

Through assessment we will establish what support is most appropriate: one to one support or group support or a combination

Support can be offered at the Coppice Centre located at Primrose Hospice, in school or in the community as appropriate.
