At Primrose, we feel it is important that you are able to build a trusting and professional relationship with our nursing team. This will enable you to gain the most from attending our Day Hospice or Clinical Nurse Specialist Clinic.

Following an initial assessment, you will be allocated a Key Nurse. They will be responsible for planning your care and evaluating progress. You will be offered one to one time with your Key Nurse every time you attend the hospice within a private consultation room.  Your Key Nurse will be responsible for communicating and liaising with your GP and other professionals involved in your care.

This time will be spent addressing symptom control issues, providing emotional support and an opportunity to discuss your care wishes and future planning.

We encourage you to contact your Key Nurse by phone should you need in between attendances and appointments. You will be provided with a contact card with their details.
