Clinic
Some patients prefer not to attend the Day Hospice. However, they can still maintain regular professional contact with the Hospice by attending the Clinical Nurse Specialists outpatient clinic.
The nurses are able to provide support, information, symptom management or refer you to other services.
The Palliative Care Consultant is employed by Hereford and Worcestershire Health and Care NHS Trust and is based at the Princess of Wales Community Hospital.
Outpatient appointments are carried out in a variety settings such as the Day Hospice and the Princess of Wales Community Hospital , following a direct referral from the Hospice Clinical Team, GP’s and hospital Consultants.
If you are known to one of the hospice team, the consultant will see you with a member of the Clinical Team at the Day Hospice.
