Referral

Primrose Hospice aims to work as part of a community team to support adults over the age of 18 who have been diagnosed with a life-limiting illness. The hospice works as part of a wider team of professionals providing palliative care across north east Worcestershire.

We offer a high quality, excellent and friendly service which responds to the needs of an individual at every stage of their condition.

Referrals can be made by GPs, consultants, clinical nurse specialists and health care professionals. We also accept self-referrals following discussions with an individual’s GP. A reason for referral is required and the urgency is required too.

We accept referrals by letter, referral form and telephone. Our aim is to make the referral process quick and easy. The head of clinical services and Day Hospice lead will access referrals and contact the patient directly.

Family members who need support would usually be referred to Primrose Hospice by a professional. Post-bereavement, family members can self-refer to Primrose Hospice. People who have experienced a sudden death of a family member who was not a patient at Primrose Hospice can access the service at any point for up to three years.
