Family Support Social Worker

Salary: £30,000 FTE / £24,000 actual

Contract Type 30 hours per week / permanent

As the Family Support Team Social Worker, you will play a pivotal role in supporting patients and families experiencing anticipatory grief and bereavement, and enabling patients and their families to achieve preferred options for end of life care.

Your responsibilities will include:

  • Providing a skilled, palliative social work service to those individuals, children, adults and families at highest risk of social, emotional and/or family breakdown during end of life care and bereavement
  • Identifying, assessing and monitoring the impact of non-curative illness, dying and bereavement on individuals and family dynamics.
  • Family interventions to promote the well-being of patients, carers and family.
  • Providing and engaging external support provision to meet the needs of patients and their families.

We are looking for someone who is proactive, organised, and driven to make a positive impact. The ideal candidate will:

  • Be a qualified social worker, CQSW, DIPSW degree or equivalent
  • Hold a professional registration with Social Work England
  • Have a post-qualification award, in relevant area, e.g. palliative care, multi-agency working, family therapy

Primrose Hospice & Family Support Centre provides exceptional care and support for people with life-limiting illnesses within North East Worcestershire. This is an exciting time to join us as we look to adapt our services and provide care to more people than we ever have before.

The Team Administrator provides a crucial role within the Family Support Team at Primrose Hospice. Often the first point of contact for our clients, you will be responsible for providing administrative support to the Coppice Team who provide pre and post bereavement services to both adults and children. This is a varied role where you will help us to provide compassionate and efficient support to our service users and colleagues.

To be able to fulfil this role, the skills and experience we are looking for in a potential candidate are detailed in the Person Specification below.

This role will be 18.75 hours a week, Monday to Friday. Due to the nature of this position, the successful candidate will be office based at our hospice in Bromsgrove (B60 3BW).

We provide exceptional care including Day Hospice, nurse led clinics, physiotherapy, living well service, complementary therapies, supportive services, including family support, counselling, and benefits advice. We host and support multiple groups open to patients, carers, and relatives, including supporting children.

We are looking for a highly motivated cleaner to work together to ensure the Hospice is cleaned to a high standard for the benefit of all stakeholders.  The ideal candidate will have commercial cleaning experience, preferably in a healthcare setting; training and support will also be given.

The cleaning team will be instrumental in helping us deliver our vision for the future of Primrose Hospice. We are looking for a candidate who can join a small team and make a big difference in supporting our service users.
