Primrose Hospice at Home

Our Primrose Hospice at Home service supports patients who are living with life-limiting illnesses and their families.

The service is designed for patients attending our Day Hospice support services who may wish to be additionally supported by Primrose Hospice in their own home.

We can:

  • Support patients in their own home by providing personal, social and emotional support whilst maintaining a connection to other Primrose Hospice services.
  • Provide patient care in a familiar environment contributing positively to their sense of dignity.
  • Connect patients at home to advanced clinical services within Primrose Hospice and the wider healthcare system – aiming to avoid hospital admissions where possible.
  • Attend to a patient, whose condition may have deteriorated, ensuring support and treatment is in place for the future.
  • Support families and carers by providing an opportunity for respite or additional emotional support.
  • Help to support patients to remain at home and potentially die at home if that is their wish.

Our Primrose Hospice at Home service will be assessed daily by our clinical and wellbeing team, ensuring we are supporting patients depending on need and service capacity.

Please note this service is available for existing Primrose Hospice & Family Support Centre patients only. There is no charge to access this service.

Primrose Hospice & Family Support Centre is an independent charity and relies heavily on support from the local community. We have to raise over £1.95 million each year to continue to provide our vital services to care for our patients and families.
